Opening Hours
How do I make an appointment?
We try to offer you an appointment with a doctor within 48 hours. Appointments are for 10 minutes and are for one patient only. You can make an appointment with a doctor, nurse or
other healthcare staff by calling 01246 432030 or by calling in person at the reception desk.
Advice by phone
We are happy to give advice over the phone. Call 01246 432030 and leave a message for the doctor or practice nurse to ring you back after surgery.
What do I do in an emergency?
Call 01246 432131. If calling outside surgery hours your call will be redirected to Derbyshire Health Care, who will advise you what you should do.
Or, you can call NHS Direct on 0845 46 47. They give advice about healthcare and illness.
How do I arrange a home visit?
Please try to go to the surgery whenever possible. If you are to ill to visit the surgery, please try to ring before 10AM. The receptionist will ask you for details of the problem
so that we can assess how urgent your visit is. We offer a social car scheme service run by Chesterfield Social Car Scheme for patients who are mobile but have difficulty getting
to the practice. The car scheme will pick you up from home, bring you to the surgery and take you home again after your appointment.

How do I get my test results?
Call 01246 439101 We cannot give your results to anyone else unless you ask us to.
If the result is complicated, we may ask you to speak to the nurse or doctor.
Please allow seven working days between having your test taken and phoning for the results
How do I get a repeat prescription?
Call 01246 439100. Between 11AM and 4:30PM you can talk to a receptionist. At other times please leave a message on the answer phone. Please quote your name, date of birth
and code number.
Or, you can put the prescription request slip in the prescriptions box in the foyer. You can also post it to the surgery and enclose an envelope with your name and address and a
stamp on it. We will then post your next repeat request slip back to you.
Please allow two working days for your request to be processed. Sometimes the doctor may not sign your repeat prescription. This is because they may want to see you first to
monitor illness and medication. When this happens, ask the receptionist for an appointment with the doctor.
How can I get a sick note?
If you are off work for more than seven working days, your employer will want a sick note from your doctor. If you are off work for less than eight working days, you should fill
in a self-certification form, which you can get from your employer or us. If you need a sick note for any other purpose, we will issue you with a private certificate, but there
will be a charge for this.
How can I register with the practice?
We can only accept patints who live in our practice area. The receptionist will be able to tell you if you live within the area. If you join our practice we will offer you an
appointment with one of the practice nurses who will review your medical history and other health advice.
